After 95 years of loving and living, cherished mother, daughter, wife and friend Phyllis Jean McCormack passed away on Friday February 4th, 2022 in Lebanon, IN.
Phyllis was born on October 27, 1926 in Frankfort, IN to the late David and Lela (Martin) Collins. She enjoyed a lively childhood with an older brother, Tom, and a younger brother, Joe. As a young woman, she attracted attention with her raven hair, bright red lipstick and lively personality. It was a trait that would last her whole life.
Phyllis married Donald Haskell in December 1946, with whom she shared a daughter, Nancy. On May 15, 1954, she married William “Bill” Weltner. Together they had five children and busy lives, including a move from Indiana to Greeley, CO in 1968. After Bill’s unexpected death in 1999, she returned to Lebanon and her Indiana roots and was fortunate enough to find a new love to be found with James “Jim” McCormack. whom she married in 2004.
Phyllis leaves behind a legacy of an amazing work ethic that she has demonstrated throughout her life. But her most enduring gift has been her unwavering and unwavering love for her children, which is passed down from generation to generation.
Survivors include her husband Jim and their six children Nancy (Kenny) Kincaid of Lebanon, IN, Kim (Dan) Jones of Fredericksburg, VA, Lynn (Scot) Hayworth of Ft. Collins, CO, Kevin Weltner from Redwood City, CA, Todd (Jennie Sue) Weltner from Boise, ID and Beth (Michael) McGahan from Orlando, FL; grandchildren, Derek (Abby), Jared, Daniel (Danielle), Tori (Zach), Wesley, Amber, Tyler, Bryce (Carly), Aidan, Ella, Madison, Mitchell and Conor; as well as great-grandchildren, Hadley, Jeremiah and Wyatt. Also surviving are Jim’s children, Mike (Ann) McCormack of GA, Julie (Chris) Thrine of CO, Jan Almond Grabil of IN, and Jerry (Babette) McCormack and Ken (Beth) McCormack of Lebanon, IN. She was preceded in death by her brothers Tom and Joe.
